Please be reminded that the forms you need to fill out and submit are Form A (Foodlane Accreditation Application Form), Form B (Foodlane Reference Form), and Form C (Statement of Commitment).

After you have completed the necessary steps and emailed the necessary forms and requirements, you will be notified to claim your food pass and foodlane vehicle pass card both with dry seal. You may also request somebody to claim the passes on your behalf provided you give them authorization letter or certificate.

We further inform you that the issuance of these passes requires no payment in any form or is free of any charges.
